Abstract
O projeto de investigação: “Senioridade Feminina e Incontinência Urinária:
Fisioterapia baseada num estudo de caso” teve por objetivo analisar e compreender o
caso clínico, à luz da evidência científica acerca da incontinência urinária feminina na
idade avançada, bem como, a abordagem da fisioterapia, numa perspetiva gerontológica.
Do ponto de vista dos objetivos o método de abordagem foi qualitativo, para
desenvolver uma investigação descritiva pormenorizada, do tipo estudo de caso.
O caso estudado foi uma paciente de 79 anos, com incontinência urinária mista
grave, seguida durante 2 anos pela Fisioterapia em Uroginecologia, do serviço de
Medicina Física e Reabilitação, do Hospital de Braga.
Esta investigação acerca da problemática da incontinência urinária permitiu-nos
compreender e analisar de forma reflexiva a abordagem da mulher sénior com IU, pela
Fisioterapia, mediante os pressupostos da gerontologia.
Com base no estudo de caso apresentado verificou-se os benefícios da intervenção
implementada, na redução efetiva das perdas urinária, no aumento do tempo entre
micções e controlo da urgência, associadas à melhoria da perceção da contração
específica e dos vários parâmetros de força dos músculos do pavimento pélvico, bem
como, nos resultados alcançados nas várias escalas de qualidade de vida e autoeficácia
aplicadas. Verificou-se ainda a relevância das entrevistas terapêuticas, que foram
permitindo uma interação salutogénica e uma individualização da intervenção.
Em conformidade com a bibliografia consultada, sustentou-se a necessidade da
prática baseada na evidência e da divulgação da intervenção da Fisioterapia, tendo esta
que ser cada vez mais holística e individualizada, particularmente no âmbito da
incontinência da mulher sénior.
This research project with the theme: “Feminine seniority and urinary incontinence: physiotherapy evidence based practice in a case study”, has the goal of analyzing and understanding through scientific based evidence the appropriate interventions in urogynacologycal physiotherapy in the elderly with urinary incontinence. When considering the goals, a qualitative research approach was used, to develop a case study. The case study was done on a 79 year old patient with severe mixed urinary incontinence, which was treated for two years by physiotherapists from the Rehabilitation Center in Braga Hospital. This research deals with the focus of this issue in the ageing population: “Feminine seniority and urinary incontinence “, and allows us to understand and analyze in a reflective way about the practice in physiotherapy (assessment and treatment) in a senior woman with urinary incontinence. Based on the study presented study, there is confirmation of the effectiveness of the intervention, mainly in the perception of the specific contraction and in the reinforcement of the pelvic floor muscles. The research indicated positive results in both the quality of life and the self-efficacy scales. The therapeutic interviews confirmed their relevance, since they play an important role in promoting salutary interactions and a more individual intervention. According to the bibliography consulted, an evidence based practice, is a need to guide physiotherapists in their intervention, and the promotion of this intervention is necessary. This practice should always be done in a holistic and individualized manner, particularly in the senior population with urinary incontinence.
